The Ohio University clinical packet, overview sheets and preceptor packet
Every Ohio University graduate practicum runs on paperwork with a fixed due date: the clinical packet, backed by the clinical overview sheets and the preceptor packet for your track. None of the three is published. The forms sit on a graduate forms page open only to signed-in Ohio University users, and the School of Nursing sends the overview sheets and preceptor packet by email on request. This guide sets out what is public, where each document lives and what to gather before you open them.

What Ohio University publishes about the packet
Less than most students expect. The Important Dates page in the Graduate Nursing Knowledge Center posts a "Clinical Packet Deadline" for each semester, and the MSN FAQ says every NP clinical runs "with an approved preceptor and an assigned clinical supervisor." What the packet contains, and exactly where it is submitted, appears on no public ohio.edu page.
The working documents are kept for enrolled students. The graduate Forms and Resources page now opens only after Ohio University's Microsoft single sign-on, and the files behind it are stored on Ohio University's SharePoint. Your copies come from there, or from the School of Nursing by email.
So read any outside description of the packet, this page included, as orientation. The version that counts is the one your course and the School of Nursing give you, and the Graduate Nursing Student Handbook explains the rules the forms assume you already know.
Three documents, three jobs
Students tend to use these names interchangeably, but Ohio University files them separately. Archived copies of the graduate forms page, captured in July 2024, May 2025 and January 2026 before it moved behind sign-in, show the titles. Only the titles: the contents were never public.

The same archived page carried the Graduate Handbook, a "Graduate Clinical Hour Completion/Extension" form and an "Affiliation Agreement Request Form Graduate." The 2024 copy also listed "Typhon Consent Forms," an "Out of Sequence Request Form" and a "Graduate Clinical Checklist," none of which appeared in 2025 or 2026. Titles change, so confirm which ones are current before relying on any of them.
Where to get your copies
- Open the graduate forms pageSigned in with your Ohio University account, go to Forms and Resources in the Graduate Nursing Knowledge Center. That is where the packets and overview sheets for each track have been posted.
- Email the School of NursingThe Knowledge Center lists graduatenursing@ohio.edu for questions about "clinical placements and paperwork." Give your track, the practicum course number and the semester, and ask for that course's clinical overview sheets and preceptor packet.
- Check TyphonOhio University says graduate students upload documents to Typhon "for program/track requirements and/or clinical placements." A search summary of the forms page mentioned packets inside Typhon, but that could not be verified, so follow your course's instructions on where the finished packet goes. The Typhon guide has more.
- Request the handbook at the same timeThe Graduate Nursing Student Handbook also comes by email from graduatenursing@ohio.edu. Read its clinical sections next to the packet.
Certificate applicants who already practice as APRNs should take step two before admission. The PMHNP certificate page says they will "more than likely" begin a clinical course in their first semester and "should have possible preceptors and sites in mind when applying." Details to gather before you open the forms
Because the packet's fields are private, prepare the facts that every preceptor and site form rests on. Faculty sign off on the preceptor and on the site separately, and a quick decision depends on facts that are exact and consistent.
Collect from your preceptor and site
- The preceptor's full name, credentials and specialty, written as they appear on the license.
- License type, number, issuing state and expiration date.
- Site name, street address and the kind of practice it is.
- A named contact at the site for student paperwork, often in education or human resources at larger facilities.
- The days and weekly hours the preceptor can commit for the whole semester.
- Whether the facility requires its own agreement or onboarding before your first day.
Then check fit. The MSN FAQ places FNP clinicals in "primary-care offices or hospital/community-based clinics" and PMHNP clinicals in settings such as "private or community mental health agencies." A site that matches Ohio University's wording for your track is simpler to approve. See preceptor requirements for who can precept each track.
State rules can matter as well. Ohio University's licensure disclosures note that in Illinois the APRN student and the preceptor must hold an Illinois RN license, and that Maryland requires out-of-state preceptors to be licensed in Maryland.
The deadline, and the clocks that run out first
Every packet is due on the School of Nursing's date for that semester: 11/09/2026 for Spring 2027 and 03/08/2027 for Summer 2027, each nine weeks before the term opens. The packet deadlines page has the full table and a plan that counts back from each date.
Two other clocks can expire sooner. If your site belongs to Cleveland Clinic, Holzer, Mount Carmel or OhioHealth, that system's application window closes before the packet does; see hospital system applications. If the facility needs an affiliation agreement, allow months rather than weeks: Ohio University's RN-to-BSN pages say one hospital group's process alone can take up to two to three months. The common slip: waiting for a preceptor's firm yes before asking for license, credential and site details. Ask during the first conversation, so the yes turns into a complete packet within days.

Questions Ohio University students ask
Are the clinical packet and the preceptor packet different documents?
Ohio University names them separately. The archived forms page filed clinical packets by course number, while the PMHNP certificate page asks applicants to request "the clinical overview sheets and preceptor packet" as a pair. Which pages your preceptor completes and which you complete is set out in the documents themselves; if the split is unclear, ask your course faculty or graduatenursing@ohio.edu.
Can I see the clinical packet before I am admitted?
Usually not, since the forms page opens only to signed-in Ohio University users. One group is invited to ask early: the PMHNP certificate page tells current APRNs to email graduatenursing@ohio.edu for the overview sheets and preceptor packet while they apply, because they typically begin a clinical course in their very first semester.
Do I need a new packet for each practicum course?
The archived titles point that way: packets were filed with specific course numbers, such as FNP 6840, 6841 and 6842, and the School of Nursing sets a packet deadline every semester. Plan to submit before each practicum term, and ask faculty whether a preceptor who stays with you from one course to the next needs fresh paperwork.
What happens if my preceptor changes after I submit?
Tell your course faculty and the School of Nursing straight away, because approval covers a specific preceptor and site. Ohio University's public pages do not describe a change process, so check the Graduate Nursing Student Handbook and your syllabus. If a preceptor has pulled out entirely, preceptor dropped out walks through recovering the semester.
Does the RN-to-BSN use a clinical packet?
Not under that name. RN-to-BSN students upload a Preceptor Agreement Form for each clinical course in a Pre-Clinical Canvas course and wait for approval before they may enroll. The NRSE 4600 page puts that deadline at approximately 90 days before the course begins. The RN-to-BSN page lists the current dates.
